Update some doc file names

Remove "cdc" from directory names in the partials directory.
Update cross-reference links and include statements to no longer have "cdc".
Update file names to have these prefixes:
assembly-
con-
proc-
ref-
This is required to fetch upstream content and automatically build the downstream user guide.
