Add database and table to MySQL event metadata

      Currently the only way to determine the names of the original database and table from which a change event emanated is to parse the topic name using the convention. This is not always deterministic, though, since the topic names may change (especially in the future with DBZ-121).

      The MySQL connector should add the database name and table name as fields in the message envelope.
