GOAL

• Allow admins and developers to create new DB instances in the OCP console

• As an admin, I would like to have the ability to create a new DB instance for my users
• As a developer, I would like to have the ability to create a new DB instance for my application 

REQUIREMENTS

• Which information do we need to show in the creation flow for each DB provider?

DESIGN STORY

We are adding the ability for an admin or a developer to create, view and manage their database instances on OCP.  This feature should be available in the database instance list view.
