Description
When selecting a value from the drop down lists below the instance name field, the text of the instance name is deleted

Steps to reproduce:

1. Add a Cockroach DB provider account
2. Go to Database Access --> Configure --> Create database instance
3. Select Database provider - Cockroach DB --> Insert name under 'Cluster Name' field
4. Change the value in one of the drop down list fields like 'Hosting Plan', 'Cloud Provider' etc.

Expected result:
The text inside the cluster name field will remain as it was

Actual result:
The text under the cluster name field disappears and needs to be inserted again
