Thus far, from time to time we've made form/UX changes specific to providers that were meant to hold us over until we had an opportunity & bandwidth to revisit all providers. We're about to rework several forms, giving us perfect opportunity to clean up where needed. This initiative should capture that effort.

We'll begin with an ADR identifying what information we'd like to collect & how to do it in a generic way that's as future-proof to new providers/forms/updates as we can manage.

Off the top of my head, we should consider at the least: fields, field names, descriptions, placeholders, help text, validation rules, but not limit or constrain ourselves to these items
