Remove all references to Developer Launcher and Minishift from the Node.js 16 Runtime Guide.

• Remove each of the following sections from the guide:
  • 1.2. Application Development on Red Hat OpenShift using Developer Launcher
  • 2. Downloading and deploying applications using Developer Launcher
  • 5. Example applications for Eclipse Vert.x
  • I. Glossary

• Search for any remaining references to "Launcher" and "Minishift" and remove these references also.

• Removing the example applications chapter also requires removal of the following:
  • Paragraph about example applications towards end of section 1.1. 
  • 1.3.3. Introduction to example applications
  • Cross-reference link to the REST API Level 0 example at the start of the new Chapter 2.
  • Cross-reference links to the REST API Level 0 example in the new sections 3.1.1, 3.1.2, 3.2.1, 3.2.2, and 3.2.3
  • Appendix F. Proficiency levels
