Problem Description:

1. Dynamic surveys - Connect to a database containing your information or run another template to return json encoded information back to survey to be used for execution or some other kind of external connectivity
2. Add checkboxes and maybe even sliders to surveys
3. The ability to create your own window to customize the look/feel of the survey
4. For text type boxes/pull downs allow for an option to use a label to be shown to the user but then use the data behind the label to be used within the Workflow or Template
