Under Line items in the invoice add application name to differentiate the costs

Current Behaviour
Under Line items in the invoice are listed:

• fixed fees with the application plan between brackets, i.e. Fixed Fee (Basic)
• Refunds with the application plan between brackets, i.e Refund (Basic)
• Methods, i.e Hits
• Metrics, i.e test

When an Account has multiple applications with fixed fees, methods and metrics the invoice line items will appear with only this informatio, for example:

It is difficult for the Provider and Developers to understand what has been charged for each application.
 
Expected Behaviour
Show the application name and under the application list the corresponding fixed fees and variable costs, with a subtotal for each application:
