Add support for connecting directly to the Jaeger collector

Currently our opentracing integration via the Jaeger client connects to the Jaeger collector through the agent but some users prefer to connect directly to the collector. This is supported in Jaeger so should be possible with the Jaeger client/lib we use for this integration.
