Workaround Description:
1. Enable Account Plans
2. Delete all the account plans except one
3. Disable Account Plans
Steps to Reproduce:
1. Enable Account Plans on Usage Rules Page
2. Create an additional Account Plan other than the Default one
3. Disable Account Plans
4. Account approval required disappears

Description

In /site/usage_rules/edit if more than 1 account plan exists and Account Plans are disabled then Account approval required disappears from Usage Rules page.
