Failed to create a deployment module when using a module name that was removed previously.

The deployment service failed to create a module with a name that was previously removed, e.g. Create and deploy a destination (topic) with a name "myTopic" and then remove the "myTopic" destiation. Now create another destination using the same name "myTopic" and the deployment service will fail to create the deployment module.

A test case (testCreateAndRemoveAndCreateTopic()) has been added to the DeploymentServiceUnitTestCase class to demostrate this error.
