Re-unify monitoring content in the operations/monitoring.adoc file

In an earlier release, content about monitoring Debezium was modularized and split into individual files that are stored in the partials directory. Now that we have a utility that can split the files when we fetch upstream content for re-use downstream, we can put the monitoring content back into one upstream file. We'll update our fetch script to split the content when we fetch it.
