Connect schema name is wrong for SQL Server

    Description

      Connect schema name for table is constructed as catalog + schema + tableId. All databases but SQL Server has either catalog or schema empty so it is not included. SQL Server has both componenets present but catalog is a constant for Debezium point of view so it is superfluous part.
